At the bottom of the ELCA opening page under “Resources” click on “Forms” in the middle column “For Congregations” click on the second one down “Ministry Site Profile Form (formerly the Congregational Mission Profile)”. Be sure you download the “Formatta Filler-Full” (on the right) before downloading the “Ministry Site Profile” (under creating an account). You will not be able to download the MSP without the Formatta Filler-Full.
